Supplemental Accidental Death and Dismemberment
Insurance
You may purchase supplemental AD&D insurance from 1 to 8 times your base pay
(rounded to the nearest $1,000), up to $1,000,000. Rates are based on the amount of
insurance you purchase.
Accident insurance can help you pay expenses if you or your eligible dependent is
seriously injured or killed in a covered accident.
A covered accident is a sudden, unforeseeable, external event, resulting directly and
independently of all other causes, in a covered injury or covered loss which occurs
while coverage is in force. To help survivors of severe accidents adjust to new living
circumstances, we will pay beneits according to the chart below.
If, Within 365 Days of a Covered Accident, Bodily Injuries We Will Pay This % of the Benefit
Result in: Amount
Loss of life, or total paralysis of upper and lower limbs, or 100%
loss of any combination of 2: hands, feet, or eyesight, or
loss of speech and hearing in both ears
Paraplegia (paralysis of both lower limbs) 75%
Total paralysis of both upper limbs, total paralysis of upper 50%
and lower limbs on 1 side of the body, or loss of hand,
foot, or sight in 1 eye, or loss of speech, or loss of hearing
in both ears, or severance and reattachment of 1 hand or
foot
Loss of 1 arm or 1 leg, total paralysis of 1 upper or lower 25%
limb, or loss of all 4 fingers of the same hand, or loss of
thumb and index finger of the same hand
Coma 1%
If the same accident causes more than 1 of these losses, we will pay only 1 amount, but it will be the largest
amount which applies.
