        Access to Care:
        •   We have vaccines at multiple locations and to call the COVID vaccine line for an appointment.

        •   COVID-19 vaccine update (as of June 3, 2021):
               o Who we’re vaccinating:
                         Veterans of all ages, Spouses, and Caregivers, DHS, VBA, National Cemetery staff,
                          and VA Employees
                         Thus far, we’ve given more than 69,314 doses of the vaccine to Veterans and
                          employees
                         Over 89% of all VA Employees have been vaccinated
               o Where and when we’re vaccinating:
                         Main Campus – 650. E. Indian School, inside the hospital
                              •  Scheduled Appts: M-F, 630 am to 330 pm, and Sat 8-12 (Pfizer)
                              •  Specified Walk in Hours: M-F, 630am to 6pm, and Sat 8-12
                              •  J&J clinic: Walk-in, M-F, 8am to 6pm
                              •  Inpatient vaccination upon discharge, second vaccine is scheduled at
                                 Veteran’s choice of location
                         Outpatient clinics, point of care
                              •  Specialty
                              •  GeriPACT
                              •  ALS
                              •  Spinal Cord Injury
                         Southeast Clinic – drive-through (3285 S. Val Vista Dr., Gilbert)
                              •  Monday- Saturday, 7am to 1pm
                         Northwest Clinic – inside the clinic (13985 W. Grand Ave. Suite 101, Surprise)
                              •  Monday – Friday, 10am to 4pm
               o COVID Vaccines for Veterans in the Community
                         Veterans may get vaccines in the community.  It’s important to receive the second
                          dose at the same location of the first dose. For instance, if a Veteran gets the first
                          dose at the VA, he or she must receive the second dose at the VA. If a Veteran
                          receives the first dose in the community, he or she must receive the second dose in
                          the community.
               o Vaccine Virtual/Telephonic Outreach:
                         Phoenix VA employees are contacting all unvaccinated Veterans to offer an
                          appointment. As of 6/3/2021, we have reached 24,395 Veterans. Approximately 20%
                          of those contacted had their vaccine elsewhere, 40% declined vaccination, 30%
                          unable to reach, and 10% agreed to an appointment for vaccination
