        Guest Resolution Form


        What is it?
        Fill out, and submit, this form when you directly handle or address a guest complaint.



        What do I really need to know?
        Choice’s Customer Relations team handles all complaints on the first call, without in-
        volving the hotel. A case management fee will be assessed for valid complaints within
        the hotel’s control. Filling out the Guest Resolution Form lets the Customer Relations
        team know how you have dealt with the issue directly and may help you avoid addi-
        tional fees.
