giving placement of 22 wagons from Road No. 5 to Goods siding Via Main line, 9
Wagon front trolley (SE 30079372401BCNE) from engine derailed near point no.
28A.(An Joint departmental inquiry committee consisting of Asst. scale Officers i.e.,
ADEN/KRNT,ADSTE/S & ADME is nominated to conduct inquiry).

Primary:
Sri. E.Anjaneyulu, Goods LP/KCG – Imposed a penalty of “With holding of Annual
increment for a period of one year(NR) issued on 01.2.19}.

Secondary & Blame worthy: Nil.

Cause of the derailment: Jerk exerted by the LP during shunting.

5. On 24.12.18 at about 08.40hrs, While backing empty rake of T. No. 17025 Exp(SC-
MUGR) into carshed/MLY on loop line3, fourth Coach No.01450 GS SC from Engine
rear trolley all wheels derailed.

17.7.19)

Secondary :
1. P.Vikranth, ALP/SC/SC Divi. --- (with holding of two sets of PTOs issued on 10.7.19)
2. P.Yettaiah, Pass.Guard/SNF/SC Divi.- Retired on superannuation on 31.12.18.
3.G.Saikrishna, Pts.man A/MLY/SC Divi. – (with holding of Three sets of privilege passes
issued on 28.3.19).
4.Jagsir Singh, Pts.man A/MLY/SC Divi – (with holding of Three sets of privilege passes
issued on 28.3.19).

Blame worthy : Nil

Cause: empty rake formation was not released by the staff properly

6. On 07.01.19 at about 07.30hrs, at ECS/MLY shed, while performing MMTS empty
rake shunting taken two routes and second coach SC TC 411018 rear trolley
derailed at point no. 14 and front trolley derailed near point no.12.

Primary:
1.N.Sukumaran, SSE/PPIO/ECS/MLY – „CENSURE‟ issued on 22.2.19.
2.K.Nagabhushanam, Tech-I/ECS/MLY - {Imposed a penalty of “With holdingof Annual
increment for a period of one year(NR) issued on 13.2.19}

Secondary :
M.Surender, Shunter/ECS/MLY – Exempted by the committee, considering the facts
noted by the Sr.DME.

Blame worthy: Nil.

Cause: Improper setting of points by Shed staff.
