Please note for 2022, as in past years, all associates need to confirm their personal information and designate a life insurance beneficiary.
You can review your personal information, make changes and enroll in benefits online at www.mywatermarkbenefits.com or call the
Watermark Benefits Call Center toll-free at 833-281-0450.

Be sure to have your dependent information (names, social security numbers and dates of birth) handy when you call! You will be
required to provide proof of dependent status if you are enrolling/adding dependents for the first time to your Watermark benefits, so be
prepared with approved verification documentation. You will find a full list of these documents on www.mywatermarkbenefits.com
website.
Important Notices and Plan Information

You will find regulatory notices, health care reform notices, summary plan descriptions and other details concerning your health and
welfare benefits posted on mywatermarkbenefits.com. You may request hard copies of any of these documents by contacting Human
