CIGNA LOCALPLUS NETWORK

LocalPlus helps you to save money by giving you just the right mix of health benefits at the right price. It’s a cost-
efficient plan designed to help you control rising health care costs – without sacrificing the quality you want and expect.

In 2022, ITC Service Group will introduce the Cigna LocalPlus Network, limited to doctors, specialists, and hospitals
based on where you live. If you enroll in the Cigna LocalPlus HDP with HSA option, your coverage will be provided
through the LocalPlus Network.

What You Need to Know About Cigna’s LocalPlus Network
 In your local area, or when you are in any LocalPlus Network area, you must receive care from a healthcare
professional or facility who is part of the LocalPlus Network to receive coverage.
 If you choose to go outside the LocalPlus Network when a provider in the network is available, your services will not
be covered, and your out-of-pocket expense will be higher than what you would pay for in-network care.
 The only care that will be covered outside of the Cigna LocalPlus Network is Emergency Care.

To see if your doctor is part of Cigna’s LocalPlus Network or to find care:
 Go to www.Cigna.com and click “Find a Doctor, Dentist or Facility”.
 Under “How Are You Covered” click “Employer”.
 Enter your location in the search box, then select the type of provider to search.
 Confirm your location under “I Live In” and click “Continue”.
 Choose “Cigna’s LocalPlus” from the list of medical plans to see providers in the LocalPlus Network.
