LIFE & DISABILITY COVERAGE




        Life Insurance is an important part of your financial wellbeing, especially if others depend on you for support. New in
        2022, ITC Service Group will provide Basic Life and AD&D Insurance for employees as well as voluntary insurance options
        for employees and their dependents.

        Basic Life and AD&D Insurance
        ITC Service Group now provides all full-time eligible employees with Basic Life and AD&D coverage of $25,000 at no cost
        to you. This coverage is automatic; you do not need to enroll.

        Voluntary Life and AD&D Insurance

        You may choose to purchase additional Life and AD&D coverage for yourself and your dependents at affordable group
        rates. Rates are based on age and the coverage level chosen. Please note that you must purchase coverage for yourself
        in order to purchase coverage for your dependents. It is the responsibility of the employee to remove coverage once
        dependents are no longer eligible.

                                                  Voluntary Life and AD&D Coverage
                                          1x to 5x your base annual salary, rounded to the nearest $5,000 ($30,000 minimum)
         Employee
                                          Up to a $1,500,000 maximum
                                          Increments of $25,000 (not to exceed 50% of employee voluntary coverage amount)
         Spouse
                                          Up to a $100,000 maximum
                                          You have two life insurance coverage options for your eligible dependent children under the age of
         Child(ren)                        19, or to age 25 if a full-time student
                                          $5,000 or $10,000 per child (not to exceed 50% of employee voluntary coverage amount)


        Voluntary Disability Insurance
        Disability insurance provides income replacement should you become disabled and unable to work due to a non-work-
        related injury or illness. ITC Service Group provides eligible employees the opportunity to purchase voluntary disability
        coverage. You must enroll in this benefit.
                                                    Voluntary Disability Benefits
                                          Covers 50% of your base pay up to $1,200 per week
         Short-Term Disability (STD)
                                          Elimination period: 15 days
                                          Covers 50% of your base pay up to $6,000 per month
         Long-Term Disability (LTD)
                                          Elimination period: 180 days
