Active & Fit Program through HMSA
Active & Fit is a comprehensive exercise and fitness program that provides members with fitness club
membership and the self-care tools to take charge of their health. The Active & Fit Program offers the
following:
◼ A fitness facility membership at one of a national network of participating fitness clubs
and exercise centers (enrollment in the Fitness Facility Program requires a $100 annual
membership fee).
◼ Home Fitness Program for those members who choose not to join a participating fitness
facility (enrollment in the Home Fitness Program requires a $10 annual membership fee).
◼ Access to ActiveandFit.com.
◼ Access to Active & Fit quarterly e-newsletter.

Tips for enrolling with Active & Fit
◼ You'll need to link your plan to find your discount if you create a My Account. Be sure you
have your HMSA membership card with you.
◼ Have a credit card ready. You'll set up your gym payment information when you enroll on the
Active & Fit site.
◼ Your gym enrollment may take a few days—print, a Guest Pass Letter, to hit the gym right away.
