Risk Insurance
Basic Life, Basic AD&D, and dependent life insurance benefits are paid for you by Skyworks. Short and
Long-Term Disability are also provided at your expense. Please assign a beneficiary on the Manulife
enrollment form and return it to HR Global Services. Skyworks also provides Business Travel Insurance
to provide protection while traveling domestically or internationally. The following pages include more
information on each of these benefits.
Life Insurance
Basic Life Insurance - Skyworks provides free life These are a general summary of benefits. For a
