Eligibility
Employees who work at least 30 hours per week are eligible for the benefits described in this guide.
Note: Variable hour employees are tracked per Affordable Care Act guidelines which will ultimately determine eligibility.
Your benefits will take effect on August 1, 2023 and will remain in effect until July 31, 2024. If hired after August 1, 2023, your benefits will become effective
on the first of the month following 60 days (waiting period might vary due to job class). The following dependents are also eligible:
• Your legal spouse
• Your children up to age 26
Changes to your benefits
Generally, you may only make or change your existing benefit elections as a new hire or during the annual open enrollment period. However, you may
change your benefit elections during the year if you experience an event such as:
• Marriage, divorce or legal separation • Qualification by the Plan Administrator of a child support order for
medical coverage
• Birth of your child or your domestic partner’s child
• New entitlement to Medicare or Medicaid
• Death of your spouse or dependent child
• Qualification by the Plan Administrator of a child support order for
• Adoption of or placement for adoption of your child
medical coverage
• Change in employment status of employee, spouse/domestic partner
or dependent child
You have 30 days from the qualified life event to make changes to your coverage. Depending on the type of event, you may need to provide proof
of the event, such as a marriage license. If you do not make the changes within 30 days of the qualified event, you will have to wait until the next open
enrollment period to make changes (unless you experience another qualified life event).
