Life and AD&D Insurance
You have the option to purchase additional life insurance coverage. New hires can elect
Supplemental Life Insurance for themselves and their spouses up to the Guaranteed Issue
amounts listed with no medical underwriting. Employees who did not elect coverage when

they were originally hired can still take advantage of this coverage but will have to provide
Evidence of Insurability information and be medically underwritten.
Evidence of Insurability
If you apply for additional life insurance, you may be required to fill out a medical questionnaire called
an EOI form. The form should be returned directly to The Standard. The amount you elect that is in
excess of the Guaranteed Issue limits will not be effective until you are approved for coverage
Benefit
$10k Increments up to $300K max
Employee
Guarantee Issue: $80K when first eligible
$5k Increments up to 100% of Employee amount or $150k Max
Spouse
Guarantee Issue: $10k when first eligible
Dependent Child Guarantee Issue: $10k
