Need assistance finding an in-network provider? Go
to www.horizonblue.com, then;

1. Click on “find a doctor”
2. Pick “Not a member—continue as Guest”
3. Use the drop down menu “Choose a Plan to
Start”, select your corresponding plan
1. Horizon OMNIA
2. Horizon Advantage EPO
4. Enter your ZIP code, or City/State
5. Search by Doctor, Specialty or Physician

Select to continue as guest:
Step 1: Select Vision
Step 2: Choose the Plan from the drop
down and click Continue to Davis Vision
Step 3: Follow prompts to search for a
specific provider or do a geographic search
