LIFE AND ACCIDENTAL DEATH & DISMEMBERMENT (AD&D) INSURANCE COVERAGE
Life insurance is an important part of your financial security, especially if others depend on you for support.
Accidental Death & Dismemberment (AD&D) insurance is designed to provide a benefit in the event of
accidental death or dismemberment.
CPC Behavioral Healthcare provides Basic Life and AD&D Insurance to all eligible employees through
Prudential. These benefits are available at no cost.
SHORT TERM AND LONG TERM DISABILITY INSURANCE COVERAGE
The goal of the disability plan is to provide you with income replacement if you become disabled and are
unable to work due to a non-work-related illness or injury.
Short Term Disability benefits are available through Unum. Long Term benefits are available through
Prudential.
