MEDICAL PLAN INFORMATION
FINDING A PROVIDER
Need assistance finding an in-network provider? Go to www.horizonblue.com, then:
1. Click on “Find a Doctor”
2. If you are already a Horizon member, Click on “Sign In”. If you are not yet a member, please
Click on “Continue as Guest”
3. Click on the type of provider you are searching for “Medical, Behavioral Health, Dental or Vision”
4. Use the drop down menu “Plan Selected”, select your corresponding plan:
i. Advantage EPO
ii. POS
iii. Direct Access
5. Enter your Zip Code, or City/State
6. Search by Doctor, Specialty or Facility Name
HOW TO OBTAIN A REFERRAL (POS Design 10 Plan ONLY)
• If you need to see a doctor other that your Primary Care Physician, you will need to
obtain a referral.
• Your Primary Care Physician needs to add the referral into the Navinet System
(Horizon’s claims system). In the case that the Navinet system is not working, Horizon
will send your Primary Care Physician a paper copy of the form to be filled out.
• Even if your Primary Care Physician hands you a paper referral to bring to the
Specialist’s office, your Primary Care Physician would still need to add the referral into
Horizon’s Navinet system.
• The paper referral that the Primary Care Physician hands you is for the Specialist’s
records, not for billing purposes. It is the responsibility of your Primary Care Physician
to add the referral to the Navinet System.
• Confirm with the Specialist that they have received all of the information needed
before attending your appointment (if you do not do so, you will be charged for the
services provided).
