BENEFIT BASICS




        CPC Behavioral Healthcare offers a comprehensive suite of benefits to promote health and financial security for you and
        your family. This booklet provides a summary of your benefits. Please review it carefully so you can choose the coverage
        that is right for you.

        BENEFIT BASICS
        You are benefit eligible if you work at least 30 hours per week for health, dental and vision benefits, and 30 hours per
        week for Life, LTD, and AD&D benefits for employee only coverage. Benefits begin the first day of the month following
        your start date. You may enroll your eligible dependents for health, dental and vision coverage, as follows:

               Legal spouse
               Children up to age 26
               Dependents over the age of 26 who are disabled and approved by the insurance carrier(s)

        The benefit year begins on 7/1 and ends 6/30. Outside of the annual open enrollment period you may only change
        coverage if you experience a qualifying life event.

        QUALIFYING LIFE EVENTS
        You may change your benefit elections during the year if you experience an event such as:

              Marriage/ Divorce or legal separation
              Birth of your child
              Adoption of a child
              Change in employment status of employee, spouse or dependent child
              Qualification by the Plan Administrator of a child support order for medical coverage
              Entitlement to Medicare or Medicaid
              Declined Medicaid/CHIP (You must notify HR within 60 days)

        You must notify Human Resources within 30 days of a qualifying life event. Depending on the type of event, you may
        need to provide proof of the event, such as a marriage license. If you do not contact Human Resources within 30 days of
        the qualified event, you will have to wait until the next annual enrollment period to make changes (unless you
        experience another qualifying life event).

        BENEFIT COSTS
        CPC largely contributes to the health care costs of its employees. Below outlines the benefits cost sharing and tax
        treatment of benefit payroll deductions.
