Life and AD&D Insurance

_ You have the option to purchase
additional life insurance coverage.
New hires can elect Supplemental
Life Insurance for themselves and
their spouses up to the Guaranteed Benefit
Issue amounts listed with no medical
underwriting. Employees who did $10k Increments up to $300K max
not elect coverage when they were Employee Guarantee Issue: $80K when first
originally hired can still take eligible
advantage of this coverage but will
have to provide Evidence of $5k Increments up to 100% of
Insurability information and be Spouse Employee amount or $150k Max

medically underwritten. Guarantee Issue: $10k when first
eligible

If you apply for additional life insurance Dependent Child Guarantee Issue: $10k

you may be required to fill out a medical
questionnaire called an EOI form. The
form should be returned directly to The

Standard. The amount you elect that is in
excess of the Guaranteed Issue limits
will not be effective until you are

approved for coverage
