Find a UHC Provider

UHC SignatureValue Advantage HMO

If you’re planning to enroll in the SignatureValue Advantage HMO (Canopy Health) plan, it’s important to
know that you must select a Primary Care Provider (PCP) when you enroll. If you do not select a Primary Care
Provider at enrollment, a PCP will be automatically assigned to you. Directions to search for a In-network PCP
have been provided below. Enrollees will be required to enter in their selected PCP’s Provider ID number.
Please be sure you’re entering the Provider ID number and not the NPI number. NOTE: the 10-digit Provider
ID always starts with a “0” and the NPI number starts with a “1”.

Locating your HMO Primary Care Provider (PCP) Number

Before beginning your enrollment, follow the below steps to locate your UHC PCP Number:

1. Go to www.welcometouhc.com/casvadvantage
2. Scroll down and select >Search the network: UnitedHealthcare SignatureValue Advantage

3. Then, enter the address, city & state, or zip code where you would like to search for PCPs
4. Lastly, select a PCP and locate the providers 10-Digit Provider ID number (Not NPI number)

NOTE – PCPs need to be located within 30 miles of where you live or work. Also, some PCP’s participate in
more than one medical group. The medical group you select will determine where you receive referrals for
specialty care. If you select a “Canopy Health” provider, you can seek referrals from any Medical Group
within the Canopy Health Network.

UHC Select Plus (CA) and Choice Plus (NON-CA) PPO
Find an In-network Provider

To find a doctor, hospital, lab and other providers in your network on myuhc.com, follow these steps:

California residents:

1. Go to www.welcometouhc.com/selectpluswest
2. Scroll down and select >Search the network: The Select Plus Plan (west)
3. Then, enter the address, city & state, or zip code where you would like to search for providers

Non-California residents:

1. Go to www.welcometouhc.com/choiceplus
2. Scroll down and select >Search the network: The Choice Plus Plan
3. Then, enter the address, city & state, or zip code where you would like to search for providers

Additionally, members can locate an in-network provider by calling the number on the back of their health
plan ID Card or searching using the UnitedHealthcare mobile app available on iPhone and Android. See the
