You may be able to purchase additional coverage for yourself (subject to any underwriting or other
proof of insurability requirements) in multiples of $10,000 (up to $300,000). Subject to policy
terms, you can also purchase coverage for your spouse in multiples of $5,000 (up to $150,000) and
for your children in the amount of $10,000.
The Plan also provided for benefits in the event of a loss in connection with an accidental death or
injury (“AD&D”). Your AD&D benefit is equal to your Life Insurance benefit. You may apply
to purchase additional AD&D coverage up to the amounts of any additional Life Insurance benefit.
Your coverage will be reduced once you reach age 65, until at age 75 your benefit will be 35%.
You have a limited right to convert the coverage to an individual policy if your coverage under
this group plan terminates.

Long Term Disability Insurance Benefits
Eligible Employees: All active L.L.C. owner-employees or non-seasonal employees of the
Employer regularly working a minimum of 65 hours per pay period who are citizens or residents
of the United States or Canada, and all active non-seasonal coaching or training employees
working a minimum of 65 hours per pay period who are foreign nationals under contract with the
Employer to perform services in the United States (“Full-Time Employees”).
Employee Contributions: You are not required to contribute towards the cost of this coverage.
Insurance Effective Date: June 1, 2020
New Hire Effective Date: Coverage will become effective on the first of the month coinciding
with, or next following, your date of hire; provided that you are an Eligible Employee on that date.
Date Coverage Terminates: The date on which you terminate employment.
Benefits:
This program pays 60% of your pre-disability earnings (minimum monthly benefit of $100;
maximum monthly benefit of $12,500) once you have been unable to work in your own occupation
due to a disability for a period of 90 days. If your pre-disability annual earnings are less than
$90,000, you must be unable to work in any occupation for the benefits to continue after 24 months
of benefit payments. In general, benefits may continue until age 65, but special rules apply.
Benefits will be reduced to the extent you earn certain other income while disabled.

Vision Benefits
Eligible Employees: All active employees working a minimum of 35 hours per week (“Full-Time
Employees”).
