discounts on eye wear purchases. The plan also Eye Exam & Contact
offers Lasik discounts. Lens Evaluation & Once Every 12 Months
Fitting
You vision plan provides coverage for routine eye Eye Exam
exams and pays for all or a portion of the cost of (includes dilation as

indicated)
