You can choose from three Blue Cross Blue Shield of FL medical plan options:

• Traditional PPO Plan
All plans offer comprehensive health care benefits, including coverage for prescription drugs and free preventive care services.
No matter which plan you choose, you’ll always pay less when you use in-network providers. To see if your doctor, hospital or

Wellness Incentive (Rally)
For employees enrolled in the Blue Cross HSA Plan or the Blue Cross HRA
Plan—you and your spouse or domestic partner may complete wellness
activities to earn money to be deposited into your HSA or HRA.
Some examples of these activities include taking a health assessment,
completing an annual physical, and more!
