We are pleased to offer all Full-Time Employees a choice between three Major Medical plans with Sana
Benefits. Sana is partnered with HealthSmart PPO, RBP Medical Partners and ZERO cost with deductible
waived for many providers
NEW Dental & Vision, with Lincoln Financial with enhanced benefits including increased dental annual
maximum including Warby Parker added to the vision offering all Len's cost and enhancement at a flat
$15.00 Copay.
Affinity Neurocare Contributes to the cost for Medical, Dental, and Vision.
Affinity also provides all Full-Time Employees with a flat amount of 100% Employer Paid through Lincoln
Financial Life Insurance with matching Accidental Death & Dismemberment (AD&D) at NO cost to you with
Lincoln Financial based on class. We provide though Lincoln Financial employer paid Short-Term and Long
-Term Disability.
NEW for employees and dependents is low cost Voluntary Life & AD&D term life with Lincoln Financial with
Guaranteed Issue of $30,000 for employees and $10,000 for spouses with NO health questions and cannot
be turned down. Children Life is $10,000 for a flat monthly cost of $1.44 regardless of how many children
are covered.

self-service Website (Employee Navigator), anytime 24/7. This personalized website offers detailed infor-
mation on your current plan, benefit summaries, cost breakdowns, provider and network search tools,
