helps our employees stay healthy and feel secure while maintaining a work/life balance.
Catalyst Urban Development is pleased to offer two medical plans to meet your individual
needs. We also provide our employees with employer 100% paid basic life and AD&D,
Employee Assistance Plan (EAP) for you and family members.
Catalyst Urban Development pays 100% of the cost of the employee only Silver Option
S667CGC medical plan. The Employer Silver Option contribution goes towards the Gold
Option G9K4CHC H.S.A. medical plan. Our employees can also purchase medical cover-
age for their dependents (at an added cost).
Please retain a copy of this guide in order to review benefits, cost information, and contact
information. This booklet will not serve as an ID card or verification of benefits at a providers
office, but can provide assistance if needed. For Additional contact numbers and
