Affinity Neurocare is committed to a comprehensive benefit program that helps our employees stay healthy and feel
secure while maintaining a quality work/life balance.
MEDICAL is renewing with Blue Cross Blue Shield (BCBS) and will continue to offer four comprehensive medical plans.
Our medical plans offer a variety of Doctor Copays, Deductibles, and Out of Pocket Maximums. (Medical plans come
with Virtual Visits, BCBS has Wellness programs for healthier lifestyles including Telahealth. Affinity Neurocare Con-
tributes a large amount of Employees Cost of the base plan and We contribute majority to all of Employees Dental
and Vision and 100%.
VOLUNTARY DENTAL is with Dental Select. Both dental plans provide 100% preventative services with annual deducti-
ble and a large calendar year maximum. This year Employees Dental is covered at 50%! Your dental has both in and out
of network benefits.
VOLUNTARY VISION is offered through Superior Vision. New Enhanced benefits and this benefit is covered at 100% for
employees.
VOLUNTARY LIFE, LTD and AD&D is going to change this year, you can carry over your benefits and stay with Lincoln
and keep it with you know matter where life takes you. We will be helping you do that. We are also looking at other
options, however please transition over to Lincoln and if we find you a better alternative we can always make the
switch. We are working on the what plans you can transition and will let you know cost and how to do it.
Please retain a copy of this benefit guide for a brief review of your employee benefits, helpful information, costs, and
necessary contact information. Our benefit guide will not serve as an ID card or verification of your benefits at a
provider’s office, but it can provide assistance if needed.
