contact information, and network provider look up. You can also find helpful (how to) videos, provider
links, and forms. Please retain a copy of this guide for future reference, including coverage benefits,
cost information, and Employee Navigator Login Instructions on pages 12 & 13 Note that this booklet
does not function as an ID card or proof of benefits for providers but can assist you with relevant infor-
