The UHC Drug List is updated twice a year. Check www.myuhc.com for the newest Preferred Drug List (PDL) and to confirm how your prescription drugs are covered or changes in your prescription drug copay. The www.myuhc.com website is the best source for up-to-date information about the medications your pharmacy benefit covers, possible lower-cost options, and cost comparisons.
Voluntary Supplemental Life
Supplemental Life Insurance coverage is available to provide you and your family with additional financial security. As a new hire or during Open Enrollment 2017, you had the opportunity to elect coverage for you, your spouse, and your children without evidence
of insurability. For Open Enrollment 2018, you will be given the option to increase your Supplemental Employee Life insurance coverage by $10,000 not to exceed $200,000 without evidence of insurability.
Voluntary Supplemental Long-Term Disability
Supplemental Long-Term Disability coverage is a great way to expand the long-term disabilty benefit. If you previously declined to participate, you will now be given an opportunity to enroll in 2018. It is important to note that this enrollment is subject to a pre-existing condition limitation within the first 12 months of coverage under the plan.

You will receive a new card if you:

» Or newly elect to participate in Flexible Spending Accounts (FSA)
