What can I expect when I submit a LOA claim request?
After you create a new LOA claim request, you will receive a letter from The Standard Insurance Company that includes:
• The requested dates of your leave
• Information about your leave entitlement (and disability benefits if applicable)
• Any documentation that we need to support your leave and/or disability claim and the date it is due
• Your assigned leave number, which will also be given to you online after you create our claim
When I call to report my absence, what questions will I be asked?
Besides answering other questions about your absence, you’ll be asked to provide the following information:
— Employer name: Ventura Foods, LLC — Group Policy number: 759063
— Employee ID number
— Last day you were at work
— Reason you’re requesting leave
— For Salaried Employees: Physician’s contact information (name, address, phone and fax number)
  Things to Gather BEFORE You Request A LOA
Depending on the type of leave, you will be asked to provide some basic information. Having the following information readily available when you report your absence to The Standard Insurance Company will speed up the process:
Personal Information
Name, address, telephone number, and the last four digits of your Social Security Number.
Job Information
Job title, job description, workplace location and address, work schedule, supervisor’s name and telephone number, date of hire, and last day worked.
Illness/Injury Information
Nature of the illness, how, when, and, if applicable, where the injury occurred, the date your disability began and when the disability commenced.
Provider Information
Name, address, telephone number, and fax number for each treating provider.
