Adverse Drug Reactions

                   Definition:

                   "is a response to a drug that is noxious and unintended and which occurs at doses normally
                   used in humans for the prophylaxis, diagnosis or therapy of disease or for the modification of
                   a physiological function. " WHO, 1970 Since 2012"

                   ADR also could result from medication errors and uses outside the terms of the marketing
                   authorisation, including the misuse and abuse of the medicinal product.

Medication errors are more common than adverse drug events, but result in harm less than 1% of the time.
About 25% of adverse drug events are due to medication errors
