Role of the pharmacist in ADR

COMMUNICATION MANAGING
The basic principles
Is the patient's drug therapy appropriate?
Do the benefits of therapy outweigh the possible risks?
Is a safer alternative available?
Is there a potential for drug – drug or drug – disease interactions?
Is the patient particularly vulnerable?
Does the drug have a narrow therapeutic window?
Is the patient on the lowest effective dose?
At-risk patients
Extreme of age: (elderly patients- neonates and infants)
Gender (pregnancy)
Concurrent drug usage
Concurrent disease, e.g. Respiratory disease
pharmacokinetic variables, e.g. Renal or Hepatic function, thyroid dysfunction
pharmaceutical factors, e.g. Nature of dosage Form or excipients
Genetics, e.g. Glucose-6-phosphate dehydrogenase (G6PD) deficiency.
Patients with a history of ADRS/allergies.
