Failing to see dangerous complications, particularly the harmful interaction of
drugs.
Failing to warn patients of the dangerous side effects of drugs (incorrect
directions)
Marketing defective or unsafe medications.
How to Prevent Errors
Ensure sufficient staffing.
Check that the prescription entry is correct.
Clarify any unclear information.
Check prescriptions thoroughly.
Provide patient counseling.
It should be the goal of every pharmacist to ensure there are no medication errors.
