Conclusions
• Pharmaceutical care reflects a systematic approach that
makes sure that the patient gets the right medicines, in
the right dose, at the right time and for the right reasons.
• It is about a patient-centered partnership approach with
the health care team accepting responsibility for
ensuring that the patient’s medicines are as effective as
possible and as safe as possible.
