GROUP CRITICAL ILLNESS INSURANCE COVERAGE

Critical Illness insurance coverage provides a cash benefit to the policyholder when an insured person has a
covered illness event. Additional benefits beyond option 1 and 2 can be accessed online in employee
navigator.

Eligibility:
All Active Full Time Employees working a minimum of 20 hours per week.
