WHEN CAN I MAKE CHANGES TO MY
BENEFITS?
During the plan year you may only make changes if
you experience a qualified change in status.
Qualified changes in status include:
» Marriage, divorce, legal separation, domestic
partnership status change,
» Birth or adoption of a child, change in child’s dependent status, commencement or termination of adoption
proceedings
» Death of spouse, child or other qualified dependent
» Spouse reaches age 65 and is covered by Medicare
» Change in spouses or domestic partners benefits or employment status
» Change in residence due to an employment transfer for you, your spouse, or your domestic partner
» A court order
Any changes you make must be consistent with the change in status and supporting documentation will be
required. You must make the change within 31 days of the date the event occurs.
