Care Planning
Whole books are written about care planning. Action Pact’s workbook, Changing the Culture of Care Planning: a person-directed approach, gives further guidance regarding the culture change practices of “I” format and narrative care planning and how the federal regulations support each. The term “care plan” assumes care is needed. The term and concept is very much in line with the medical model. The focus on care makes sense for medical and physical needs, but not for the rest of the person. A person-centered care plan whoudl include a person’s whole life needs.
