An example of a care plan that identifies the need for TLC might look like:
Activity Interest
Because my condition does not allow me to respond to you like I once did, I could use extra attention and human contact.
As I have always been an outdoorsy person, I would like to go outside as often as possible even if it’s cold and even if for just a short while.
My other lifelong interests include Bible study, scrapbooking my family photos and looking at my photo albums.
Goals
I will verbalize, smile, clap or show in some way that I enjoy and appreciate you:
1) taking me outside,
2) helping me continue to
study my Bible,
3) helping me look at my
family photos each week.
Approaches
Special one-to-ones have been designed to meet “Mrs. Johnson’s” interests and goals and will be provided or arranged by staff
four days a week. They include taking her outside, reading to her from the Bible and looking at her scrapbooks with her in addition to the following which totals at least eight special one-to-ones per week:
A volunteer has committed to take me outside once a week.
My family has committed to take me outside twice a week.
Please bundle me up with my heavy coat, mittens, scarf and hat. Thank you.
My pastor has committed to come see me once a week.
Please help me watch my favorite church service on TV on Channel 7 Sundays at 11a.m. and Tuesdays at 7p.m.
Please play my Bible recording CDs when I’m in my room.
Please keep my TV on the Christian station 41.
Please keep my radio on station KLOVE.
My family has also committed to looking at photo albums with me when they visit. They know who people in the photos are and can remind me.
Group activities I enjoy are church services, some larger community events and anything with children. Please invite me to those and I can indicate with a nod whether I want to take part.
