D. The composition, functions and responsibilities of the Supply Management Office.
The Supply Management Office is composed of: an Administrative Officer, Administrative Assistants;
and Administrative Aide.
The Supply and Property Unit of Isabela State University is responsible in providing services related
to supplies, property and equipment.
Its specific functions are the following:
1. Accepts deliveries of supplies, materials and equipment based on the Approved
Purchase Request (APR), Purchase Order (PO), and request for the conduct of
inspection.
2. Issues and distributes available supplies, materials and equipment to requesting
offices/colleges.
3. Prepares and maintains Inventory Custodian Slip for semi-expendable,
Requisition and Issue Slip for supplies and materials, Acknowledgement Receipt
for Equipment for all Property, Plant and Equipment (PPE).
4. Conducts physical inventory of all PPE of ISU, and consolidates the same for the
preparation of Annual Inventory Report.
5. Identifies unserviceable equipment and waste materials for disposal and prepares
inventory and Inspection Report for Unserviceable Property and Waste Materials
for submission to the disposal committee.
6. Signs clearance of property accountability for all employees and students at ISU.
F. There is a duly approved Procurement Management Plan of the Program being surveyed.
FUND SB 164 and FUND SB 101 (End User/Unit: Joan T. Ruiz, DPA.- CBAPA Dean). It covered office
supplies, equipment, furniture, construction, repair and maintenance.
