New for the 2021 Plan Year

We are continually looking to implement new benefit programs that will provide value to you and
your families. That is why we are pleased to tell you that new for 2021, our health plans will be
administered by a company called Centivo.

Centivo is a new type of health plan administrator that offers higher quality care and lower costs to
colleagues and their families by directly contracting with local healthcare providers you know and
trust. In addition, Centivo offers traditional health plan options with national network access via Cigna.

You’ll have the choice of three medical plans
for the coming plan year*:
