WORKPLACE HEALTH, SAFETY AND SECURITY
OFFENSES
Gambling, lottery, or any other game of chance on the employer’s premises during working hours. C Possession of deadly weapons or explosives inside the company premises. D
Malicious mischief or any act aimed at causing alarm, panic or confusion inside company premises. B-D
Violation of a safety rule or safety practice resulting in loss or damages to company and/or injury to co-worker.
C-D Deliberate or careless conduct endangering the safety of self or other employees. B-D
Harboring a contagious disease which may endanger the health of other employees without notifying the proper company officials.

Acts of creating / contributing to unsanitary conditions. B Failure to report an accident/injury promptly to proper company officials. B
Knowingly admitting an unauthorized person or persons into any locked or restricted area of the workplace or building.
C-D
Refusal to abide by or failure to comply with other company, building and departmental policies such
as but not limited to adhering to safety procedures and moving, tampering, unauthorized use, and A-D destruction of safety devices.
