If your company is hiring servers through the Hotel (recommended), you do NOT have to fill out the
remaining training information below.
If you are not using Hotel servers, the On-Site Outlet/Booth Manager or another company employee
designated to assure responsible alcohol beverage service must have completed certification from an
organization approved in one or more states.
