Once you click on the presentation
in question, it drills further down in to
prescriptions given out by the practice.
The caveat here is that it looks at all
prescriptions written by the practice,
not just those for the dispensary. Here
it will make some suggestions around
cost and how to best reduce the drugs
cost to the NHS. It is worth noting at
this point that all of the suggestions
around cost are based on cost to
the NHS on which items they are
reimbursing the practice for, not cost
to the practice itself.
To work out how they get to these
savings figures, we can then use the
DM+D browser (dmd-browser.nhsbsa.
nhs.uk) to get an idea of why they are
suggesting a switch and the costs
that then drive the saving value. As the
image shows, a generic Sulfasalazine
prescription is reimbursed at £58.28
before clawback. In order to generate
the cost savings shown, we would
need to explore the reimbursement
prices for the other actual medicinal
product packs listed. Clicking on the
Salazopyrin- EN tablet takes you into
the pack information. Here you can see
the reimbursement for prescriptions
written as Salazopyrin EN will be
reimbursed at £8.43, a whopping
saving from the point of view of the
NHS of nearly £50.00 per prescription.
For the practice however, this could be
left as a generic prescription to gain
the higher reimbursement value.
Another useful part of the DM+D
browser is the ability to identify
PA items, particularly if they are
uncommon or it is the first time you
have seen them. In this example, while
it is an Insulin we all know is a PA, the
personally administered flag gives you
clear sight of what items are considered
by the PPA as a PA and will therefore
receive the automatic reimbursement
as required.
On top of identifying any PA items, you
can also use it for identifying appliances
that could be sent out to your DAC of
choice, reducing the practice drugs cost
and potentially increasing profitability
through the agency fees. The 2
examples show that they are allowed as
appliances in the drug tariff.
