BENEFITS OPTIONS
Holidays • 10 holidays paid each year
Vacation • 0-4 years: 11 days
• 5-9 years: 15 days
• 10-14 years: 20 days
• 15-19 years: 21 days
• 20 years and longer: 25 days
Sick/Personal Time • Non-exempt employees accrue 48 hours per year or as mandated by local/
state regulation
• Exempt employees are on an honor system
Sabbatical • 30 day paid sabbatical after 5 years of service
Merit Reviews & Increases • Employees are reviewed annually in April
Discretionary Bonus Plan • Determined annually and paid based on company performance.
Educational Assistance • Available to full-time employees upon 90 days of hire
• 100% reimbursement up to $5,250 annually for a grade of A, B, or C
• No limitation on schools as long as it is job/degree related and from an
accredited school
Medical Plan • Plan administered by HealthComp
eligible upon 1st day of hire • Anthem Blue Cross PPO Prudent Buyer Plan (CA only)

• Employee Only: $49.42 biweekly
• Employee + Spouse: $176.20 biweekly
• Employee + Children: $126.17 biweekly
• Employee + Family: $227.99 biweekly
In-Network Highlights:
• Deductible: $600 individual, $1,800 family
• Office Visits: $25 co-pay
• Specialist: $40 co-pay
• Hospitalization: 80% after deductible and $200 co-pay
• Out of network coverage also available
Prescription Drugs • Generic: 100% after $10 co-pay
• Preferred: 100% after $40 co-pay
• Non-preferred: $70 co-pay
Other Medical Benefits • Chiropractic/Acupuncture benefits: $25 co-pay
• Integrated Behavioral health (in network):
• Inpatient for mental health/chemical dependency: 80%, no deductible; $100
co-pay per hospitalization
• Outpatient for mental health/chemical dependency: 100%, no
deductible, $25 co-pay
• Maternity Support Program
• Employee Assistance Program: 3 free counseling sessions
every 6 months, up to 5 per year for each issue type.
