RISK MANAGEMENT - USING THE SPA








          SPECIALIST TECHNICAL SERVICES



                                                                       ● A new SPA is required for all work activities.





                                                                       ● SPA is completed by the work crew - Experience and Knowledge. Must be




                                                                                 checked by the person supervising the task.




                                                                       ● SPA is completed at the task location prior to work commencing.





                                                                       ● All members of the crew must Sign their Name at the bottom of the form.




                                                                                 to Show they were Present, Understood and will Comply with the SPA and



                                                                                 were Involved/Contributed in the completion of same.





                                                                       CHANGES TO THE SPA





                                                                       ● If a new member joins the crew, the supervisor must explain the S.P.A. to



                                                                                them and have them sign the form.





                                                                       ● If the work you are carrying out changes you must stop work and discuss




                                                                                what changes you must make.




                                                                       ● Change the SPA accordingly, and if necessary………





                                                                                                                                                                Write a new SPA!
