expense. To help offset this expense, your Employer offers a dental insurance plan to eligible
employees. This plan encourages you to seek preventive dental care.
Full-time employees may join the dental insurance plan on the first of the month following their
date of hire.
A booklet containing the details of the plan and eligibility requirements may be obtained from
Human Resources or found within Paycor.
Upon termination, you may be entitled to continuation or conversion of the group dental
insurance plan in accordance with the terms of the policy and/or applicable state law. For more
information, contact the Human Resources Department.
VISION INSURANCE
100% paid by the Employee
Full-time employees may join the vision insurance plan on the first of the month following their
date of hire.
A booklet containing the details of the plan and eligibility requirements may be obtained from
Human Resources or found within Paycor.
Upon termination, you may be entitled to continuation or conversion of the group vision
insurance plan in accordance with the terms of the policy and/or applicable state law. For more
information, contact the Human Resources Department.
TERM LIFE INSURANCE
100% paid by the Employer
Regular full-time employees are provided with term life insurance coverage including accidental
death and dismemberment. Coverage begins on the first day of the month following the date
the employee becomes a regular full-time employee. The amount of coverage per employee is
up to a $25,000 maximum. If your employment is terminated, you can convert to an individual
life insurance policy through the insurance carrier by submitting an application and the first
premium within 31 days following the last day of the month termination occurs. Otherwise,
coverage ends on the last day of the month termination occurred.
SUPPLEMENTAL LIFE INSURANCE AND ACCIDENTAL DEATH AND
DISMEMBERMENT
100% paid by the Employee.
