Human Resources. This should clearly identify the problem and outline a course of
corrective action within a specific time frame. The employee should clearly understand
both the corrective action and the consequence (i.e., termination) if the problem is not
corrected or reoccurs. The employee should acknowledge receipt of the warning and
include any additional comments of their own before signing it. A record of the
discussion and the employee's comments should be placed in the employee file in the
Human Resources Department.
Employees who have had formal written warnings may not eligible for or receive
reduced salary increases, bonus awards, promotions or transfers.
POST RESIGNATION/TERMINATION PROCEDURES
Exit Interview
An exit interview may be given to a terminating employee on the employee's last day
of employment and for arranging the return of Company property.
Benefits
Benefits (Life, Medical, Dental, Vision, Allstate) end on your last day of employment, [or,
last day of the month in which your last day of employment falls]. An employee, unless
dismissed for gross misconduct, has the option to continue Medical Benefits in
accordance with the Consolidated Omnibus Budget Reconciliation Act ("COBRA")
regulations.
Employees may choose the continuation or waiver of comprehensive medical
coverage under COBRA. COBRA documentation will be mailed to the employee. See
COBRA section for more details.
Final Paycheck
Employees leaving the Employer must return office keys, corporate credit cards, etc.,
before their final paycheck can be issued. This final paycheck will be mailed or direct
deposited during the next normal pay period.
Retirement Plan
If an employee is in the retirement plan, information regarding this plan will be mailed
to the employee.
