B. Transferees must pass the Cagayan State University – College Admission Test (CSU-CAT)
                           and meet the required percentile score of 88.
                           C. Transferees must submit the following requirements:


                              1.  Transfer credentials / records / certificate of grades
                              2.  Eligibility of courses and grades obtained with a General Weighted Average of 90 or
                                 above
                              3.  Certification of Good Moral Character
                              4.  NBI Clearance or Police clearance
                              5.  Admission slip signed by the University Registrar
                              6.  Marriage contract (if applicable)
                              7.  Laboratory test results required for new entrants
                              8.  Other requirements for the first-year level applicants.

                           D. Transferees shall start with the first-year level of CSU BSN Curriculum.




                       VI. Admission Policies for Shifters from Non-Nursing Courses

                          A. If a chosen applicant decides not to proceed with the admission process, a shifter from non-
                          nursing courses may be accepted.


                          B. In order to be accepted, shifters from institutions other than Cagayan State University must

                                  fulfill the following requirements:

                              1.  Transfer credentials / records / certificate of grades
                              2.  Eligibility of courses and grades obtained with a General Weighted Average of 90% or
                                 above
                              3.  Minimum CSU-CAT Rating of 88  percentile

                              5.  NBI Clearance or Police clearance
                              6.  Admission slip signed by the University Registrar
                              7.  Marriage contract (if applicable)
                              8.  Laboratory test results required for new entrants
                              9.  Other requirements for the first-year level applicants.




                                                              Article III

                                             RETENTION, ELIMINATION, DISMISSAL,
                                         LEAVE OF ABSENCE AND CROSS ENROLMENT



                       I. RETENTION POLICIES

                          A.  If a student fails in a General Education/major/professional course for the first time, the
                              student will be allowed to retake the course.

                    B.    A  student  who  fails  the  same  course  twice  shall  be  automatically  removed  from  the  BSN
                          Program.

                          C.  For incoming third year students, they must pass the screening and battery exam.
