Supplemental Questionnaire
The purpose of this questionnaire is for you to identify your qualifications and experience in specific job-related areas. Your responses, along with your résumé and cover letter, will be reviewed and evaluated to make the initial determination of which applicants are best qualified for the position.
1. Describe your experience working with different software platforms, to include, but not limited to ADP (or similar systems), Microsoft Office, and online application systems. Include in your response what software programs you are familiar with, your level of expertise (beginning, intermediate, advanced), and how you have used each of these software programs throughout your employment.
2. Please describe your supervisory and/or lead responsibilities. For each employer where this is applicable, please include the specific lead and/or supervisory duties performed, the number of staff supervised, the classifications of the staff for which led and/or supervised, as well as the dates of employment (mm/yyyy to mm/yyyy).
