Additional Benefits

         Basic Life/AD&D – Employee Only – Anthem Blue Cross (100% Employer Paid)
         The Master’s University & Seminary provides basic life insurance at no cost to you. Your life & AD&D
         benefit amount is 1 times annual earnings up to $150,000. Minimum benefit is $20,000. Benefits
         receive a 35% reduction at age 65 and a 50% reduction at age 70.


         Optional Life/AD&D – Anthem Blue Cross (Voluntary - 100% Employee Paid)

          •  Employee benefit – in increments of $10,000 up to $300,000 not to exceed 5x base annual salary.
             Evidence of Insurability needed for amounts over $100,000, if you are a new hire. All amounts are
             subject to Evidence of Insurability for exsiting employees.
          •  Spouse benefit –increments of $5,000 up to $150,000 not to exceed 50% of employee amount.

             Evidence of Insurability needed for amounts over $25,000. All amounts are subject to Evidence of
             Insurability for late entrants.
          •  Child benefit (15 days - age 26)  – increments of $1,000 up to $10,000, not to exceed 50% of
             employee amount.
          •  Benefits receive a 35% reduction at age 65; 50% reduction at age 70
          •  Optional AD&D is automatically provided to anyone enrolled in Optional Life insurance; amount is
             the same as the elected Optional Life amount.


         Long Term Disability – Anthem Blue Cross (100% Employer Paid)
         Replaces 60% of your monthly income up to a Max of $6,000/month, after you have been disabled for
         90 days.

         Resource Advisor – Anthem Blue Cross (100% Employer Paid)
         Anthem’s Resource Advisor provides trained professionals to assist you with personal and workplace
         issues. The program is voluntary and confidential; only your Employee Assistance Program professional
         will know you have called. This program is paid for by The Master’s University & Seminary and also
         includes assistance for your immediate dependent family members.


         Helpful resources include: financial planning, legal services, identity theft recovery, and counselors who
         can help with stress, parenting, anxiety, grief, and much more.


         •   By phone: Call 1-888-209-7840.
         •   In-person: You can call to set up face-to-face sessions and then schedule appointments directly with
             your counselor.
         •   Video visit: You can talk with a counselor from the convenience of your home or wherever you have
             internet access and privacy using LiveHealth Online. To set up a LiveHealth Online visit, call Resource
             Advisor. You will receive details about how to schedule a visit, along with a coupon code that gives
             you LiveHealth Online visits at no extra cost to you.
