What is the Difference Card?
The Difference Card is a benefit funded by your employer that helps you save money on your medical costs.
The Difference Card, similar to a Health Savings Account, helps you obtain reimbursement for the cost of
office visit copays, deductibles, emergency room visits, and coinsurance expenses if you enroll in either the
Kaiser or Anthem HMO plan. However, unlike a Health Savings Account, you may only use your debit card for
certain office visit copays. For all other medical services, you must submit a claim to The Difference Card for
reimbursement. When you are billed by your HMO medical group, you can seek reimbursement from The
Difference Card based on the type of service. In order to be reimbursed, you must follow the below steps:
• Present your Anthem or Kaiser insurance card to the hospital or medical facility or provider.
• Your Insurance Carrier will pay the provider based on pre-negotiated rates and contract terms. You will
then receive a bill from the facility for any amount owed.
• Obtain your EOB (Explanation of Benefits) from your HMO medical group by logging into your Medical
Group’s website.
• Submit your EOB along with a Reimbursement Form to The Difference Card.
• The Difference Card will issue a check or will direct deposit funds for the reimbursement amount due as
determined by your Difference Card plan.
• Once you receive the reimbursement, pay the balance owed to the medical facility or provider.
PLEASE NOTE: It is important to forward all insurance carrier deductible and coinsurance EOBs to The
Difference Card, even when no Difference Card Reimbursement is due. These EOBs are entered and used to
calculate any future eligible reimbursements.
There are many different ways you can submit your claim to The Difference Card:
Online
• Visit www.differencecard.com and login to your account.
• Click the SUBMIT CLAIMS to add a claim for Reimbursement.
Mail your claim
Send your EOB and Reimbursement form to The Difference Card:

Use the Difference Card mobile app
• In the DC Mobile App, click Claims in the home menu.
• Tap the plus (+) sign in the upper right hand corner of the Claims screen.
• Choose SUBMIT A CLAIM. A brief form will display, prompting you to enter all relevant details and upload
a receipt and/or picture of your EOB.
• Click the preview button. Review the details, and when ready, tap submit.
