Your Benefits At-A-Glance
MEDICAL
The Master’s University & Seminary offers several options for health insurance. Choosing the coverage that
best meets your and your family’s needs involves reviewing our benefit offerings. Each of the plans we offer
has different levels of copays, deductibles, out-of-pocket maximums and provider networks. To make the right
choice, you need a basic understanding of the different types of plans that we offer. Brief descriptions of our
plans are listed below.
Kaiser Permanente HMO Plan – The Kaiser HMO plan includes 100% in-network coverage for most care
authorized by your primary care physician after an office visit copay. For more information, please visit

Anthem Select & Traditional HMO Plan – The Anthem Select & Traditional HMO includes 100% in- network
coverage for most care authorized by your primary care physician after an office visit copay. For more

Anthem Blue Cross PPO HSA – The Anthem Blue Cross PPO HSA plan offers coverage with the freedom to
choose your own doctor. The plan features two levels of coverage so you and your family can receive the care

DENTAL
The Master’s University & Seminary eligible employees may select one of the two United Concordia (UCCI)
dental plans. Both plans provide coverage so that you and your family can receive the care needed to maintain
good health. Your dental coverage is an important health benefit and it is important to fully understand your
plan details. Keep reading for a brief description of your options.
UCCI Dental DHMO – The United Concordia Dental HMO dental plan provides coverage for most preventive,
basic and major services with no annual deductible and no annual maximum. Orthodontic services are also
covered under the DHMO plan.
UCCI Dental PPO Options (LOW and HIGH)– The PPO dental plans allow you to receive care from the dentist of
your choice and includes extensive coverage for preventive, basic, major and orthodontic care. Choose from a
low option or high option depending on the benefits you wish to have. The better the benefit, the higher the
monthly premium.
VISION
Anthem Vision Plan – The vision plan features a 12-month frequency allowance on exams, lenses and frames.
Savings are maximized when you choose an eye doctor that is in the Anthem network. Please review the
benefits in this brochure for additional details.
ADDITIONAL BENEFITS
Additional benefits including life and AD&D coverage, long term disability coverage, FSA, Pet Insurance, and a
403(b) program are available to eligible employees of The Master’s University & Seminary. Please continue
