Find a Health Care Provider

PHCS

PHCS is a PPO network for medical doctors utilized by health plans in order to provide a broader
choice of healthcare professionals offering discounted services to their members.
*PHCS providers are not a requirement of the plan.

Two Ways to Find What You Need

There are two ways to find a network provider:

1. Contact BAS at 800-843-3831 to locate a contracted PHCS provider.
2. Visit https://www.multiplan.com/webcenter/portal/ProviderSearch (no login required)
There are four easy steps to locating your provider on the Multiplan website.
Step 1. Choose a Network (PHCS then Practitioner Only)
Step 2. Use the text box to specify what you are searching for (Internal Medicine,
Pediatrician)
Step 3. Set your location (zip code) and begin your search.
Step 4. Receive your results and refine as desired.

What to Do When You Visit a Provider
Present your Health ID Card and remind his/her office to call BAS (800.523.0582) to verify
eligibility and benefits. Instructions are on the back of the ID Card under “Eligibility Verification”.

What Is BAS?
BAS is the Third-party Administrator for your Health Plan. This means BAS processes and pays claims, and
is responsible for eligibility and customer service.

What Is 6 Degrees Health?
6 Degrees Health is a reference based pricing vendor which eliminates the Preferred Provider Networks
PPO network for facility claims, allowing you to seek care at any hospital you choose. This plan allows your
employer to manage the ballooning cost of healthcare while still continuing to provide quality benefits to
employees and their families.

Where Should Your Provider Submit Your Claims?
Claims must be submitted to the address on the back of your ID Card under “Medical Claims
Submission”. Claims are submitted Benefit Administrative Systems, LLC PO Box 2920 Milwaukee, WI
53201-2920 EDI Payor ID 36149.
