A Identify Priority
Unwanted Events
As a starting point, a historical review of the global
data on all fatalities at Holcim was performed.
During the exercise, close attention was also given to
catastrophic / multi-fatality events and factored into
the analysis methodology and weighting system. Contact with Coal Mill Contact with Material Structural Mobile Equipment Fall from Height
Hazardous Energy Explosion Hot Meal Engulfment Liquid Fuel Fire Collapse Incident
The data revealed that 85% of fatalities at Holcim
related to just 8 ‘Unwanted Events’. These events were:
Contact with Hazardous Energy, Mobile Equipment
Incidents, Material Engulfment, Fall from Height,
Structural Collapse, Coal Mill Explosion, Liquid Fuel
Fire, and Hot Meal Contact - the Fatal Repeats!
Contact with Coal Mill Contact with Material Liquid Fuel Fire Structural Mobile Equipment Fall from Height
Hazardous Energy
Engulfment
These became the ‘Priority Unwanted Events’ Collapse Incident
Explosion
Hot Meal
(P.U.E.s) and the starting point for the global journey
in Critical Control Management at Holcim.
The selection of P.U.E.s was only the first step in this
broader overall approach to managing critical risks.
